- Please type or print the form in ALL CAPITALS.
- MAIL your registration form with a nominal check payable to JORNADA CELG or your bank deposit receipt (ABN AMRO REAL Bank, Ag. 0549, 6714852-1) to: Secretaria Executiva - Caixa Postal nº 5540 - CEP 90410-007 - PORTO ALEGRE (RS).
- Participants will receive their check payment receipt together with the registration packet upon check-in at the meeting.
- Or FAX your registration form along with your bank deposit receipt to: (51) 3225 3763.
- Registration fee payment may be in up to 3 installments, provided the last installment is paid by August 10.

IMPORTANT DATES
- You may pre-register until August 13, 2004. If you are unable to do so, you may register on-site only.
- Registration forms must be postmarked on or before August 10, 2004.
