Ele contém elementos do horror, sejam derivados de clássicos do gênero (casas mal assombradas, histórias de fantasmas, zumbis, entre outros), ou sejam coisas mais específicas do próprio RPG escolhido, por exemplo, monstros lovecraftianos. É importante esclarecer que o tipo de cenário pode permear o RPG de diferentes formas: existem aqueles em que o foco principal é no elemento de terror, mas outros que podem se focar em outras coisas e, mesmo assim, terem alguns elementos de terror espalhados durante as aventuras.
